WHO Prescribing Guidance

Indications · Administration · Dosage · Precautions

Reference information summarised from WHO prescribing guidance. Expand each item below — clinical use must always follow the locally approved product insert.

🎯Indications▾
Treatment of constipation (osmotic laxative that increases stool water content and softens stool); treatment and prevention of hepatic encephalopathy (portal-systemic encephalopathy) — lowers blood ammonia by acidifying colonic contents. Lactulose is listed on the WHO Model List of Essential Medicines.
💧Administration▾
Oral, with water, juice or milk to improve palatability; may also be given rectally (as retention enema) for hepatic encephalopathy when oral intake is not possible.
⚖️Dosage▾
Constipation — adults: initially 15–30 mL (10–20 g) daily, adjusted to response; maintenance 10–15 mL daily; children 7–14 years 15 mL, 1–6 years 5–10 mL, infants under 1 year 2.5 mL daily. Hepatic encephalopathy — 30–50 mL three times daily, adjusted to produce 2–3 soft stools per day.
⚠️Precautions & Warnings▾
Initial flatulence and abdominal cramps are common and usually transient; excessive dose causes diarrhea with risk of dehydration and electrolyte imbalance (hypokalemia, hypernatremia) — reduce dose accordingly. Use with caution in diabetes mellitus (contains galactose/lactose sugars). Contraindicated in galactosemia and suspected gastrointestinal obstruction.
