🎯Indications▾

Prevention and treatment of venous thromboembolism, pulmonary embolism, acute coronary syndromes and extracorporeal circulation (WHO essential medicine for unfractionated heparin).

💧Administration▾

IV bolus/infusion or subcutaneous injection; enoxaparin/other LMWHs are given subcutaneously. Rotate injection sites and do not aspirate or massage.

⚖️Dosage▾

Unfractionated heparin: prophylaxis 5,000 units SC every 8–12 h; treatment by weight-based IV infusion with aPTT monitoring. Enoxaparin prophylaxis 20–40 mg SC once daily. Adjust for renal impairment.

⚠️Precautions & Warnings▾

Bleeding risk — monitor platelet counts (heparin-induced thrombocytopenia), and avoid in active bleeding, severe thrombocytopenia or recent neurosurgery. Protamine sulfate is the antidote for heparin; warfarin requires INR monitoring.
