🎯Indications▾

Symptomatic bradycardia and AV block, organophosphate and carbamate poisoning (WHO essential antidote), pre-anaesthetic antisialagogue and antidote for cholinergic excess.

💧Administration▾

Intravenous (or intramuscular if no IV access) injection; in poisoning, large cumulative doses may be required until secretions dry.

⚖️Dosage▾

Bradycardia: adults 0.5–1 mg IV every 3–5 min, maximum 3 mg total; children 0.02 mg/kg (min 0.1 mg). Organophosphate poisoning: adult initial 1–2 mg IV/IM, doubled every 5–10 min until atropinisation (paediatric 0.05 mg/kg).

⚠️Precautions & Warnings▾

Anticholinergic effects: dry mouth, mydriasis, urinary retention, tachycardia and confusion. Caution in glaucoma, prostatic hypertrophy and tachyarrhythmia. Never use for organophosphate poisoning without adequate dosing titrated to clinical endpoints.
